Intensive Case Manager

Reports to: Director of Client Services

Classification:  regular full-time, exempt

Compensation: $48,000.00/year with generous benefits and paid time off

Position Overview:

We are seeking a Ryan White Intensive Case Manager to within the framework of the Ryan White HIV Care Program at APNH, which provides medical and support services to people living with HIV. The role of the Intensive Case Manager is to offer comprehensive and individualized support to clients with complex needs, particularly those who face significant barriers to accessing and maintaining care and those within our priority populations (MSM of color, black women and transgender individuals of color).

Key Responsibilities:

  • Comprehensive Client Assessment: Evaluating the needs of individuals living with HIV, including medical, psychological, financial, and social aspects. This involves understanding their barriers to care and determining what services are necessary.
  • Individualized Care Planning: Developing a personalized care plan based on the client’s unique needs, setting goals for health and well-being, and outlining strategies to achieve those goals.
  • Service Coordination and Referral: Connecting clients to a wide range of support services, including medical care, housing, mental health counseling, substance abuse treatment, and social services. Case managers work to ensure seamless integration of services.
  • Advocacy: Acting on behalf of the client to secure access to services and resources, removing barriers, and ensuring their rights are upheld within the healthcare system.
  • Monitoring and Follow-Up: Regularly checking in with clients to monitor progress, adjust care plans as needed, and provide ongoing support. This includes ensuring treatment adherence and addressing any issues that arise.
  • Crisis Intervention: Responding to urgent needs, such as homelessness, food insecurity, or mental health crises, and linking clients with emergency services.
  • Documentation and Reporting: Maintaining accurate records of client interactions, care plans, progress, and outcomes. This is essential for program compliance and evaluation.
  • Education and Empowerment: Providing clients with information and support to help them understand their health condition, treatment options, and how to navigate the healthcare system effectively.
  • Other responsibilities:
  • Work as an integral part of a coordinated care team.
  • Other duties as assigned.

Qualifications:

  • Bachelor’s degree in social work, public health, nursing, psychology, or a related field, or minimum of five years case management experience. Master’s degree in social work (MSW), counseling, or a health-related field preferred.
  • Strong communication skills: Ability to communicate effectively with clients, healthcare providers, and service organizations.
  • Cultural competence: Experience working with diverse populations, especially priority population listed above, and an understanding of the cultural and social factors impacting health.
  • Problem-solving and critical thinking: Ability to assess client needs, develop care plans, and troubleshoot barriers to accessing services.
  • Organizational skills: Managing a caseload of clients with complex needs requires excellent time management and the ability to keep detailed records.
  • Knowledge of HIV care and treatment: Familiarity with HIV medications, treatment protocols, and the psychosocial impacts of living with HIV.
  • Empathy and interpersonal skills: Sensitivity to the unique emotional and psychological challenges faced by individuals with HIV, along with a commitment to supporting their well-being.
  • Proficiency in electronic medical records (EPIC EMR) and case management software(CAREWare) to document and track client interactions.
  • Ability to use Microsoft Office and other general office software for reporting and administrative tasks.
  • Knowledge of New Haven community and its services.
  • Knowledge of HIV services in New Haven.
  • Knowledge of LGBTQ+ community in New Haven.
  • Culturally sensitive and linguistically appropriate, Spanish speaking a plus.
  • Strong organizational skills.
  • Ability to work independently as well as closely with others as a part of team.
  • Ability to work nights and weekends on occasion.

Community Testing & Outreach Counselor

 

Reports to: Director of Prevention Services

Classification: Full Time, Regular

Compensation: $45,500, Plus, generous paid time off, holiday leave, sick time, 403B retirement plan matching, medical, dental and life insurance. 

General Responsibilities: Community Testing & Outreach Counselor will assist in coordinating operations of our mobile testing unit and services, including scheduling, compliance, inventory control, route/location strategy, data analysis and entry, coordination with other agencies and community organizations, outreach, volunteer scheduling and retention, STI testing and linkage to care and services.

Specific Responsibilities:

  • Assist in strategizing on mobile testing route, locations, times, and special testing events
  • Drive mobile testing until to various sites across greater New Haven
  • Set up and break down of mobile testing unit fixtures and equipment
  • Adhere to maintenance and safety protocol for mobile testing unit
  • Monitor and maintain inventory of all supplies and test kits
  • Data entry, analysis, and reporting
  • Administer HIV/STI tests and provide HIV counseling
  • Provide PrEP/PEP education and referrals
  • Ensure appropriate status neutral linkage to care for all persons tested
  • Engage with partner community organizations to provide a status neutral continuum of care to the community
  • Work with other APNH program staff to establish and engage Community Advisory Board members and activities

Qualifications:

  • High School Diploma, GED, or equivalent certification. Associates degree or greater preferred.
  • Experience working in the field of HIV or public health outreach
  • Knowledge of and experience working with Greater New Haven’s diverse population
  • Valid driver’s license
  • Ability to work as a member of a team to coordinate programs and services.
  • Ability to travel within the region and work evenings and weekends
  • Cultural competence in effectively and respectfully working with others from a diverse background
  • Ability to work in person as well as remotely and across various virtual platforms
  • Experience with HIV testing and phlebotomy and willingness to perform blood draws
  • Knowledge of Social Networking Strategies and other evidence-based interventions helpful
  • Ability to maintain confidentiality
  • Must have received or be willing to receive the COVID-19 vaccination by date of hire to be considered. Proof of vaccination will be required.
  • Bilingual English/Spanish preferred
